Requirements to Process a Reimbursement of Medical Expenses

This procedure may vary from insurer to insurer. However, it is very similar in most of them.

Is it an initial procedure?

This means that it is the first time that you will enter expenses for an illness or accident, for which you should consider:

 Documents:

For each Insured or covered illness it is necessary to present:

  1. Form "Notice of accident and/or illness" (filled out and signed by the Insured).
  2. Format "medical report" (from each specialist who has provided medical care).
  3. "Accident and/or illness reimbursement" form (completed and signed by the Insured)
  4. Proof of expenses in the name of the Insured Holder or Affected Insured of legal age, such as:
    Receipts for medical fees, prescriptions, original invoices for medications, among others (it is important that you have all the tax requirements).
  1. Results of clinical laboratory tests that have served to reach the diagnosis; or follow-up for the evolution of the condition.
  2. Interpretation of studies of:
  • Imaging: Rx magnetic resonance, computerized axial tomography.
  • Cabinet: electrocardiogram, electroencephalogram, echocardiogram, etc.
  • Pathology: a histopathological study.
  1. Care log: nursing service and physical rehabilitation.
  2. Single Banking Information Format, copy of valid Official Identification and copy of your account statement indicating: name, bank, and interbank code of the beneficiary.

 


If a procedure is subsequent?

They are the new expenses that you made as a result of the illness or accident that entered the initial process.

Documents:

  1. "Accident and/or illness reimbursement" format.
  2. Proof of expenses in the name of the Insured Holder or Affected Insured of legal age, such as Receipts for medical fees, prescriptions, original invoice for medications, among others (it is important that you have all the tax requirements).
  3. Medical prescriptions (only if there is a change in medical treatment or every 6 months).
  4. Results of clinical laboratory tests or interpretation of studies.
  5. "Medical report" format (from each specialist who has given you medical care, including physiotherapists).

If it is a chronic disease, submit updated medical reports and prescriptions (most times every 6 months).

Note: The deductible, contracted coinsurance, personal expenses and expenses not related to the condition will be deducted and proof of expenses from charitable institutions will not be reimbursed.
